ATHLETIC CLEARANCE INFORMATION

WEDNESDAY May 20 -2009: 2009-10 CLEARANCE, PHYSICAL and PARENT INFORMATION NIGHT
SUNDAY MAY 29 - 2009 : ALL 2008-09 Athletic Clearance EXPIRES
1. If you plan to participate in any spring or summer school sponsored athletic program, you must clear for the 2009-10 school year.
*Your sports physical is good for 13 months.
IF your physical will expire during the course of a sport season, you will not be able to clear for that sport. Fall sports (August 19-November 28) Winter Sports (November 16-March 1) Spring Sports (March 1- May 29).
If you can not attend the Clearance Night, the athletic office is willing to clear athletes for the 09-10 summer programs and school year during regular office hours beginning Thursday May 21 until Friday June 12th at 1:00pm. We will be closed for the summer and begin clearance again on August 12th. Office hours are M-F, 8am-4pm.
You may clear for all sports at one time as long as your physical will NOT expire during the season in which you are clearing and you pay the $3.00 for each sport.
Frequently Asked Questions about Clearance....
4/30/09 - On Clearance Night, should we go to our own doctor or to the "clinic". Will there be physicians doing the exams or sports medicine students? The physicals done at the clinic on clearance night are all done by the physicians at the clinic. They do this on a volunteer basis and our sports medicine students/staff help with paperwork as needed. The $25 fee is donated back to our sports medicine program.

1. General Information Form Make sure you have all necessary signatures and current emergency and insurance
information. Physicals are good for 13 months so your current copy may be acceptable. If you chose to purchase school
insurance, forms are available in the athletic office.
2. Purchase an ASB card $25.00 @ ASB office (bring us a copy of the receipt).
3. Pay a sport fee - $3.00 per sport @ ASB office (bring us a copy of the receipt).
4. Sign the guideline form for your sport. This must be signed by both the athlete and his/her parent. This form may
be picked up outside of the Athletic Office or you may print it online. click here to select your sport
5. Sign the Athletic Code of Conduct (must only be signed once during your high school career). Incoming
Freshman and those athletes that have never participated in a sport and his/her parent need to sign this form.
