ATHLETIC CLEARANCE INFORMATION

We are sponsoring a special PHYSICAL / CLEARANCE / PARENT INFORMATION night so you can get it all done on one night. Wednesday May 21, 2008.

6:00 - 8:30pm Clearance @ WHS Main Gym Area

There will also be information meetings for parents of freshman or people new to our school district to answer questions.

 

All athletic clearance will expire as of Monday, May 26th, 2008.

*Your sports physical is always good for 13 months... it's the other paperwork that will expire and needs to be re-submitted for the upcoming summer/school year.

Any athletes that plan to participate during the Summer of 2008 or the 08-09 school year, will need to clear again.

Clearance is required to participate in any school sponsored summer program including but not limited to:

Summer Weight Lifting, Football Camp, Cross Country Camp, Basketball Open Gym, Wrestling Open Gym, Girls Soccer Camp / Open Gym and Volleyball Open gym. Our summer programs are listed (and updated) on our website.

 

The athletic office is willing to clear athletes for the 08-09 summer programs and school year beginning Monday May 19th and will continue until Friday June 13th. We will be closed for the summer and begin clearance again on August 11th. Office hours are M-F, 8am-4pm.

You may clear for all sports at one time as long as your physical will NOT expire during the season in which you are clearing.

What you need for clearance:

1. Sports Physical Form Make sure you have all necessary signatures and current emergency and insurance

    information.  Physicals are good for 13 months so your current copy may be acceptable. If you chose to purchase school

    insurance, forms are available in the athletic office.

2. Purchase an ASB card $25.00 @ ASB office (bring us a copy of the receipt).  This will be good for the 2007-08

    school year.

3. Pay a sport fee - $3.00 per sport @ ASB office (bring us a copy of the receipt).

4. Sign the guideline form for your sport. This must be signed by both the athlete and his/her parent.  This form may

    be picked up outside of the Athletic Office or you may print it online. click here to select your sport

5. Sign the Athletic Code of Conduct (must only be signed once during your high school career).  Incoming

    Freshman and those athletes that have never participated in a sport and his/her parent need to sign this form.
